EXTRUDED DISC L5-S1 RIGHT SIDE.
Anamnesis
The patient came to the clinic 04-December-2004
complaining of LBP for 10 years with right
sciatica for 1 week with left sciatica down to
all toes left foot.
On examination: The patient is in agonizing
pain, limping with exaggerated scoliotic stance.
There is weak dorsiflexion right foot -4/5,
planterflexion same foot 4/5. SLRS was 80 degrees with pain
right
side. There is hypalgesia right L5
territory.
The patient was sent for investigations and MRI
lumbar spine performed 14-December-2004 showing
extruded disc L4-5 with right foraminal
occlusion. (Sacralization of L5).
Extended right L5 foraminotomy
with removal of the right extrusion and right sided
intradiscal cleaning of L4-5. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak. Routine
closure of the wound.
Smooth postoperative recovery.
The power of the right foot improved.
She was sent to the ward.
Follow Up
The patient came to the clinic 29-December-2004
with dramatic improvement with SLRS 90 degrees
right side, but still having mild weak
dorsiflexion right foot +4/5.
The patient then came 01-September-2007
complaining of numbness left foot with LBP
for 10 days with hypalgesia left S1 root
with no motor deficit. The patient sent for
investigations, but disappeared.
Comments
There is still an estimated postoperative
disc recurrence around 7%, since the disc space is not
completely shallow.

Inomed Riechert-Mundinger System, with three point
fixation is the most accurate system in the market. The microdrive and
its sensor gives feed back about the localization.
