26-OCTOBER-2014 MASUDA HMOUD HADI 60 YEARS
EXTRUDED DISC L4-5 WITH LEFT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic 23-October-2014
complaining of LBP with left sciatica for
6 months with numbness of the left foot. MRI
lumbar spine done 13-March-2014 showing extruded
disc L4-5 with old wedge fracture of L2 and D11.
The patient is insulin dependent diabetic for 10
years with arterial hypertension for 3 years.
On examination; the patient is limping with
exaggerated scoliotic stance. SLRS 60 degrees
with pain left side. There is weak dorsiflexion
left foot -3/5.
MRI of the lumbar spine repeated the same day,
showing extruded disc L4-5 with left foraminal
occlusion. Dynamic studies were normal.
Using C-arm. the L4-5 level was identified.
Left sided foraminotomy of L5 root was achieved.
The extruded disc was removed lateral to the
left axilla. Meticulous cleaning of L4-5 from
the left.
Routine closure of the
wound.
Smooth postoperative recovery. The power of the
left foot became normal.
Comments
The patient still has an estimated
postoperative recurrence around 7%, because the disc space
is still not completely shallow.
