SEVERE REJECTION REACTION TO THE BONE GRAFT AFTER FIXATION OF L3-4-5.
Anamnesis
The patient was operated by me 30-September-2015
for spondylolisthesis with segmental stenosis
L3-4 and L4-5, during which Lifenet-Health
allograft was used. The patient showed dramatic
recovery the first 48 hours and the wound was
clean. The third postoperative day, she started
to complain of severe LBP with difficult walking
and the dressing became wit. WBC was 7.6, CRP
was 210 mg/dL and ESR was 90 mm/h. It became
clear that the patient progressed severe
rejection reaction to this allograft.
The wound refreshed. The
wound clean. All the allograft material was
removed lateral to the rods. Meticulous
irrigation with Saline was performed until all
the small fragments were washed up. Using the
index finger and palpation the pieces of the
allograft stuck to the muscles were removed.
Several times meticulous irrigation was
performed, until no allograft left behind. Routine closure of the
wound.
Smooth postoperative recovery. The patient was
sent to the ward.
Comments
The patient progressed severe rejection
reaction to the allograft. The sooner the removal of the
graft the shortest time will be the recovery.
Our policy is to avoid using the
allografts, because all the brands caused reactions, which
led to subsequent infection. Every time the companies
persuading me, that this could not happen, but here is the
last example of this tragedy. I will not use allografts
until the end of my life.
