29-APRIL-2004 MAYAMAMO MIYAN KHAN 40 YEARS EXTRUDED DISC
C6-7 RIGHT SIDE.
Anamnesis
The patient came to the
clinic 25-April-2004 complaining of neck pain
for one month. The last week got right upper
limb pain. Diabetic and hypertensive for 7 years
in treatment. MRI cervical spine performed
22-April-20044 showing extruded disc C6-7 with
right foraminal occlusion.
On examination, the patient
in agonizing pain, having neck pain when turning
the head to all directions. There is severe weak
grip and extension right hand 3/5 and right
triceps muscle 3/5. There is hypalgesia of the
right hand excluding the thumb.
Using the C-arm, the level of
C6-7 was identified and discectomy of C6-7 was
performed and the extruded pieces in the right
side were removed until the dura was seen at the
bottom of the removed disc cavity. Routine closure
of the wound.
Smooth postoperative recovery. The power of the
right upper limb dramatically improved.
FOLLOW UP
The patient
came to the clinic 11-May-2004 with clean wound
with still having weak grip right hand 4/5 and the
hypalgesia regressed.
The patient
then came 26-May-2004 having manifestation of
Kojevnikov epi attacks and she was sent for
investigations, but she did not perform them.
She came for
several problems and the last visit was
05-November-2016 with complain of neck pain and left
upper limb pain. MRI cervical spine done
03-November-2016 showed extruded disc C5-6 with left
foraminal occlusion and she was advised to perform
surgery, but she disappeared.
Comments
The recurrence rate
in cervical disc surgery is zero. Another disc
can cause de novo extrusion as in this case.

fixation is the most accurate system in the market. The microdrive and
its sensor gives feed back about the localization.
