YEARS RECURRENT DISC L4-5 LEFT SIDE WITH INTRADURAL PENETRATION.
Anamnesis
The patient was operated by me
17-April-2004 for huge PLD L4-5 with left
sided extrusion. Then she came to the
clinic 13-June-2004 complaining of sudden LBP for
1 week with
left sciatica with positive cough
sign. She is hypertensive for 7 years.
On examination, the patient in agonizing pain,
limping with exaggerated scoliotic stance with
no motor, nor sensory deficit. SLRS was
90 degrees both sides without
pain.
The patient was sent for
investigations and MRI lumbar spine performed
21-June-2004 showing huge recurrence of L4-5
left side with intradural penetration.
Neurolysis of the left L5
roots was achieved. After removing the very huge
of the extrusion, the CSF came out. Left sided intradiscal cleaning L4-5 was
performed.
The dural defect above and posterior to the
axilla was stitched with several knots by 4 zero
nylon. The patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. A pedicle of fat was
transferred from the
subcutaneous layer and covered the exposed parts
of the dura and roots to minimize postoperative
scarring. Routine closure
of the wound.
Smooth postoperative recovery. The power of both
feet normal as before surgery.
FOLLOW UP
The patient
came to the clinic 14-July-2004 with clean wound
and no motor, nor sensory deficit.
The patient
then came 09-August with new MRI done 4 days ago
showing no recurrence.
Comments
There is still an
estimated postoperative re-recurrence around 7%,
because the disc space is still not completely
collapsed.
