SEVERE LCS L2-3, 3-4 AND L4-5 WITH ARABIC MANIPULATION CAUSING FRACTURE OF L4
SPINOUS PROCESS.
Comments
The sooner the surgical decompression in
progressive lumbar canal stenosis the better the outcome.
Chiropractic manipulations could cause
fracture to the lamina, which cause confusion to the surgeon
during work.
Anamnesis
The patient came to the clinic 10-May-2003
complaining of LBP with bilateral sciatica
for 4 months after trying all means of treatment
including Arabic chiropractors.
On examination: the patient is limping with
exaggerated scoliotic stance with SLRS 40
degrees with pain both sides with almost drop
both feet and weak planterflexion both feet 3/5.
The patient was sent for investigations and MRI
done the same day showing severe lumbar canal
stenosis L2-3, 3-4, and L4-5.
Decompressive laminectomy L3,4,
upper third of L5 and lower third of L2.
Foraminotomy L2,3,4,5 roots both sides. It was
noticed that the lamina of L4 was broken due to
chiropractic manipulation. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak. Routine closure of
the wound
Smooth postoperative recovery. He was sent to the ward.
Follow Up
The patient came to the clinic 25-May-2003 with
clean wound with weak dorsiflexion both feet 4/5. SLRS
was 60 degrees with pain in both sides.
