YEARS EXTRUDED DISC L3-4 WITH LEFT DOWNWARD MIGRATION.
Anamnesis
The patient came to clinic 21-Sptember-2014
complaining of LBP for 7 years with bilateral sciatica for
2 months more the left side. MRI
lumbar spine done 13-September-2014 showing huge
extruded disc L3-4 with left downward migration
causing severe segmental stenosis. The patient
is a known diabetic for 3 years.
On examination; the patient is limping walking
bended anterior for 6 years with
exaggerated scoliotic stance. SLRS 50 degrees
with pain in the right and 35 degrees with more
pain in the left. There is weak dorsiflexion right foot
4/5 and left foot -4/5.
Endoscopic discectomy L3-4 done from the left
side using ILLICO system. The right L5 root
became lax and the disc space free of any loose
fragments.
Routine closure of
the wound.
Smooth postoperative recovery. The power of both
feet became normal.

Comments
The patient still has an estimated
postoperative recurrence around 7%, because the disc space
is still not completely shallow.
