18-FEBRUARY-2015 MITEB ESEIFER AL-RUWEDY 60 YEARS
EXTRUDED DISC L3-4 WITH RIGHT UPWARD MIGRATION.
Anamnesis
The patient came to the clinic 07-February-2015
complaining of LBP for 1 year with right
sciatica for 2 weeks down to the right knee with
agonizing pain, using crutches for 2 days. The
patient had CVA with right hemiparesis 1 year
ago, which resolved clinically completely.
CT-scan of the brain done 25-January-2015
showing old small lacunar infarction left IC
near the GPe.
On examination, the patient is in agonizing pain
limping with exaggerated scoliotic stance with
SLRS 70 degrees with pain right side with weak dorsiflexion
both
feet 4/5.
The patient sent for investigations: MRI lumbar
spine done 07-February-2015 showing huge
extruded disc L3-4 with right upward migration
with severe segmental stenosis. There is
lumbarization of S1.
Using the C-arm, the level of L3-4 was
identified. Right L3-4 partial
hemilaminectomy with removal of the right upward
migrating extrusion in several pieces until the
axilla of the right L3 was seen. Right sided
cleaning L3-4 disc space. Inspection for
remnants of extrusion was negative and the roots
hanging free. Routine closure of the wound.
Smooth postoperative recovery.
The power of the both feet became normal
immediately after surgery.
Comments
The patient still has an estimated
postoperative recurrence around 7%.
