The patient came to the clinic 19-May-2016 complaining of right sciatica for three years with LBP with neck and right upper limb pain for 2.5 years with numbness right side of the face with dysarthria. MRI cervical spine performed 21-February-2015 showing extruded disk C5-6, 6-7 with severe spinal cord compression and malacia of the spinal cord. MRI of the brain was of very bad quality.

On examination: the patient was limping with neck pain when bending the head to the right shoulder. Weak grip right hand -3/5, extension -3/5 and right triceps 2/5. There is weak both quadriceps -4/5 right, 4/5 the left, weak dorsi and planterflexion right foot 3/5 and 4/5 of the left foot respectively. There is hypalgesia below the left D2.  SLRS was 35 degrees with pain in the right with exaggerated reflexes both sides more the right with Babinski positive both side and more brisk clonus of the right foot.

The patient was sent for thorough investigations and MRI of the brain was almost normal, except for adenoid and chronic sinusitis. MRI of the lumbar area showed wide-based extrusion at L5-S1 more to the right. MRI of the cervical spine showed huge extrusion C5-6, C6-7 with severe compression of the spinal cord and malacia.

Using the C-arm, the level of C5-6 was identified and diskectomy of C5-6 and 6-7 was performed. Using Depuy cervical cages CFRP  8 mm for C5-6 and 6 mm for C6-7 small, cervical plate Skyline 2 levels 38 mm length with 1 cc bone graft and 6 fixed screws 14x4 mm fusion of C5-6-7 was achieved. All stages of surgery were guided with C-arm. Routine closure of the wound.

Smooth postoperative recovery. The power of the right upper limb and both lower limbs became normal with persistence of the clonus.  He was sent to the ward.

There is no recurrence of cervical disk after discectomy, because there is no more disc material left behind.
