RECURRENT EXTRUSION L4-5 RIGHT SIDE.
Anamnesis
The patient was operated by me for drop left
foot due to extruded disc L4-5 in
27-September-2003. Bilateral cleaning was
performed. Then he came to the clinic
09-April-2004 complaining of severe LBP with left sciatica
for 1 week.
On examination, the patient in agonizing pain,
limping with exaggerated scoliotic stance with
weak dorsiflexion right foot and hypalgesia right L5 and S1
territories. SLRS was 80 degrees right side with
pain.
The patient was sent for
investigations and MRI lumbar spine performed
the same day showing huge recurrence of L4-5
right side.
Neurolysis right L5 root with
foraminotomy right L5
roots was achieved. Right sided intradiscal cleaning L4-5 was
performed.
The patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. A free fat was
harvested in several pieces from the
subcutaneous layer and covered the exposed parts
of the dura and roots to minimize postoperative
scarring. Routine closure
of the wound.
Smooth postoperative recovery. The power of the
right foot normalized.
FOLLOW UP
The patient
came to the clinic 21-April-2004 with clean wound
and the power of the right foot normalized
and SLRS was 80 degrees both sides without pain. MRI done
during this period after surgery showing no
recurrence.
Comments
There is still an
estimated postoperative re-recurrence around 7%,
because the disc space is still not completely
collapsed.
