The patient  a known case of diabetes mellitus started to complain of LBP for 2 years and the last 3 weeks got agonizing pain in the right with drop foot with hypalgesia right L5 and S1 roots territories. Left hemiflavotomy L4-5 with foraminotomy left L5 root was done and removal of the extrusion was performed.
