The recurrence rate in cervical disc surgery is zero.

Using plates and screws will provoke a group of complications related to such technology, such as extrusion of one of the screws. In this case it is not significant, but in other cases it could need revision.

The patient is a Yemeni citizen, came to the clinic 27-August-2002 complaining of neck and left shoulder pain for 4 years intermittent with numbness of the entire left hand and the left lower limb. MRI cervical spine done 20-August-2002 showing bulge C3-4 and extruded disc C4-5 with left foraminal occlusion. The patient was sent to perform MRI lumbar spine which was done 28-August-2002 and was normal. The patient was treated conservatively. The patient then came 22-June-2003 telling that his condition is deteriorating with numb both hands and neck pain.

On examination: the patient having neck pain when rotating the head all directions. There is weak all muscles both upper limbs 4/5 except the deltoids There is hypalgesia below the the C4 dermatome both sides.

Using the C-arm and transnasal intubation, The level of C3-4 was identified and discectomy of C3-4 and C4-5 was performed until the dura was seen relax at the bottom of both removed disci. Using Scitex 2 level plate with 6 screws, fusion of C3-4-5 was achieved. Routine closure of the wound

Smooth postoperative recovery. The radicular pain decreased and the power of upper limbs improved. He was sent to the ward.

The patient came to the clinic 02-July-2003 with clean wound with no sensory or motor deficit.

The patient then came 18-December-2004 with CT-scan performed 20-September-2004 showing acceptable alignment of the construct with the upper screw extruding for 2 mm. The patient is neurologically free and sent for MRI cervical which performed 19-December-2004 showing good alignment and no spinal cord compression.
