YEARS EXTRUDED DISC L5-S1 WITH RIGHT UPWARD MIGRATION.
Anamnesis
The patient came to the clinic 23-September-2007
complaining of LBP for 2 years and right
sciatica for 10 days.
On examination: the patient has exaggerated
scoliotic stance, using crutch when walking with
agonizing sciatica. SLRS was 30 degrees in the
right and 75 degrees in the left. Weak dorsi and
planterflexion right foot.
MRI lumbar spine done 24-September-2007 showing
huge extruded disc L5-S1 right side with upward
migration.
Using Inomed Highline IOM, the operation was
performed. Foraminotomy of the right S1 root was
performed and right 1/3 flavotomy L5-S1
achieved.
The extruded disc was removed lateral to the
axilla with right-sided cleaning of the disc
space.
Smooth postoperative recovery and improvement of
the power of right foot.

Comments
The patient has the disc space still
high. This parameters increase the expected recurrence rate.
The disc was removed from a small hole in
the annulus fibrosis to decrease the possible recurrence,
and the extruded disc was out of the disc space, which was
removed in one big piece.
The epidural fat was preserved and
reflected back around the root to prevent future fibrosis
around the involved root.
