03-JULY-2014 MUHAMED SHAALAN AL-SARRAWI 18 YEARS
HUGE EXTRUDED DISC L5-S1 WITH LEFT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic 19-April-2014
complaining of LBP with left sciatica for 18
months with exacerbation the last 3 months with
numbness of the left foot. MRI lumbar spine done
04-April-2014 showing huge extruded disc L5-S1
On examination; the patient was limping with
exaggerated scoliotic stance. Positive cough
sign, SLRS was 20 degrees with pain. Weak
dorsiflexion left foot -3/5.
The L5-S1 ligamentum flavum was dissected from
the left side and reflected to the right and
anatomically preserved. Foraminotomy left S1
root. The extruded disc was removed lateral to
the axilla and left sided intradiscal cleaning
of L5-S1 disc space. The ligamentum flavum was
returned back to cover the exposed dura.
Smooth postoperative recovery.
The power of the left foot became normal.
Follow up
The patient showed strange reaction of the wound
and Cefazolin was stopped and the patient could
discharged the next day.
Comments
The estimated postoperative recurrence is
still around 7% because the disc space height is still not
shallow.
