YEARS RECURRENCE OF L5-S1 DISC WITH LEFT FORAMINAL OCCLUSION.
Anamnesis
The patient was operated by me
28-November-2013 for huge extruded disc
L5-S1 with left posterior migration. The patient
then came 12-January-2015 complaining of
agonizing left sciatica the last week.
On examination, the patient is in agonizing pain, limping with exaggerated scoliotic
stance. SLRS was 40 degrees with pain in the
left. There is no weakness of the feet, but
hypalgesia left S1 root territory.
The patient was sent for new investigations. MRI lumbar spine
with contrast done 12-January-2015 showed
recurrent extrusion of L5-S1 with small fresh
piece compressing the left S1 root. The patient
was advised to wait for one month until the
fragment start to shrink and if the pain will
not subside, then perform surgery. The patient
then came 04-February urging for surgery.
Using the C-arm, the level of L5-S1 was
identified. Neurolysis of the left S1 root. The
extruded disc was removed lateral to the axilla.
Left sided intradiscal cleaning L5-S1. Routine closure of the wound.
Smooth postoperative recovery. The left sciatica
decreased dramatically.
Comments
There is still an estimated postoperative
recurrence around 7%.
