DISLOCATION WITH BURST FRACTURE C5 WITH DENSE QUADRIPLEGIA BELOW C4.
Anamnesis
The patient was transferred from another
hospital after falling down from stair several
hours ago at the morning of 06-August-2006 with
complete paraplegia and anesthesia below C4 both
sides. CT-scan was done and showed severe
bust fracture C5 with dislocation of C5 upon C6.
Using hollow distraction with 5-6 Kg anterior
approach was done and the flail anterior
fragments of C5 were removed.
Discectomy of C5-6 was performed and the
extruded disc was removed. Using verografin the
alignment of the bony structures were
reevaluated and and further reduction was
achieved. Using the hybrid cervical miniplate
with screws, fixation of C4-5 and C6 was
performed and good alignment was achieved. The
bony fragments were returned back to place under
the construct for future bony fusion. Routine
closure of the wound.
The patient showed immediate improvement in
power of C5 and C6 roots.
Comments
The patient had severe trauma to the
spinal cord. Reduction and fixation can help the stretched,
but not ruptured roots to recover immediately.
The recovery of the spinal cord will be
governed by many factors, which will be clear in the next 9-12
months.
