L5-S1 MORE TO THE RIGHT.
Anamnesis
The patient came to the clinic 02-October-2007
complaining of LBP for 10 years after playing
football and right sciatica for 3 years.
Exacerbation of sciatica the last 5 months. MRI
lumbar spine done 30-May-2006 showing central
extrusion L5-S1.
On examination: the patient has exaggerated
scoliotic stance, limping when walking with
agonizing sciatica. SLRS was 50 degrees in the
right and 75 degrees in the left. Weak
dorsiflexion right foot. MRI lumbar spine done
02-October-2007 showing further enlargement of
the disc L5-S1 more the right side.
Using Inomed Highline IOM, the operation was
performed. Foraminotomy of the right S1 root was
performed and right 1/3 flavotomy L5-S1
achieved. The extruded disc was removed lateral
to the axilla with right-sided cleaning of the
disc space.
Smooth postoperative recovery and improvement of
the power of left foot.

Comments
The patient has the disc space narrow.
This parameters decrease the expected recurrence rate.
The disc extrusion had a protracted
course, which predict relatively persistent pain in the
right hip in the long postoperative course.
The epidural fat was preserved and
reflected back around the root to prevent future fibrosis
around the involved root.
