The patient admitted to Shmaisani hospital transferred from other hospital and he is bedridden with Foley's catheter and not moving four limbs with loss of sensation all over with difficult evaluation of his visual function and cannot obey commands. The condition started 2 months ago with rapid deterioration. MRI done recently showing glioblastoma multiforme left occipito-parieto-temporal lobes, shifting the midline structures to the right.

Wide osteoplastic craniotomy over the left occipito-parieto-temporal region with reflection of the flap to the left ear. The dura was opened and the cortex inspected for more safe transcortical approach. The tumor was identified and fresh frozen result was of glioblastoma multiforme. Using the anatomical landmarks the resection was proceeded until the posterior horn of the left lateral ventricle and involved choroid plexus were coagulated. he inferior horn was also attacked to remove those parts of the tumor. Inspection for remnant of the tumor in the cavity for pathologic tumorous changes were negative. Practical radical resection of the visible either in the MRI or visual control was achieved. The brain is lax and pulsating. Strict hemostasis with routine closure of the wound.

Smooth postoperative recovery and the patient was sent to the ICU for 24 hours observation.

The patient came to the clinic 16-December-2003 with clean wound and has profound Anton syndrome. He can walk with help. Radiotherapy and chemotherapy was recommended.

The patient came 28-March-2004 with new MRI done 16-March-2004 showing slight recurrence of the tumor with big cavity. Radiotherapy localized and WBR was performed. The patient with the family were advised to repeat investigations after 3 months, but they disappeared.

The final histologic result was glioblastoma multiforme. The prognosis is gloomy.
