LUMBAR CANAL STENOSIS D11-L4.
Comments
The aim of surgery is to prevent further
clinical deterioration. The patient came too late to promise
him improvement of the drop both feet, but he escaped from
follow up. In some cases over the years some of them regain
improvement even with this drop foot.
Anamnesis
The patient came to the clinic
19-November-2002 complaining of LBP for 6 years
with right sciatica with weakness right foot for
2 years. MRI lumbar spine performed 28-June-1995
showing bulging with stenosis at D12-L1, L3-4,
L1-2 with old fracture at L1. There was
hypalgesia right L5 root with weak
planterflexion both feet 3/5 and drop both feet
with weak abduction of the knees 4/5. The
patient was sent for investigations and came
back 27-March-2003 with MRI done 5 days ago
showing severe stenosis D11 down to L3-4 with
extruded disc L3-4 with left downward migration.
On examination: the patient is limping with
exaggerated scoliotic stance with the same
clinical picture as before.
Decompressive laminectomy
D12,L1,2,3, upper half of L4 and lower half of D11.
Foraminotomy of all running roots at these level.
Inspection of the L3-4 disc space revealed that
there is no need to violate it. Routine closure of
the wound
Smooth postoperative recovery. He was sent to the ward.
Follow Up
The patient came to the clinic 22-May-2003 with
clean wound with some improvement, but still
drop both feet.
