EXTRUDED DISC L5-S1 WITH RIGHT DOWNWARD MIGRATION.
Anamnesis
The
patient came to the clinic 06-May-2012
complaining of LBP for one year with
intermittent course. 4 days ago after coughing
got severe right sciatica. MRI lumbar spine
performed 05-May-2012 showing huge extruded disc
L5-S1 with right downward migration
On
examination, the patient is limping with
exaggerated scoliotic
stance. SLRS was 20 degrees in the right and 30
degrees in the left, shooting to the right. Dorsi and planterflexion
right foot was -4/5.
Right foraminotomy right S1
root with preservation and reflection of the
ligamentum flavum to the left. The extruded disc
removed lateral to the axilla. Cleaning L5-S1
from the right side. Inspection of the
subaxillary area revealed extrusions, which were
removed accordingly. The reflected ligamentum
flavum returned to its place.
Routine
closure of the wound. Smooth postoperative
recovery. Normalization of the power of right
foot.

Comments
The patient still having
estimated postoperative recurrence around 7%,
because the disc space is still not shallow.
