18-MARCH-2015 MUHAMED FAREED AL-NASHARTI 43 YEARS
HUGE EXTRUDED DISC C6-7 WITH LEFT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic 12-February-2015
complaining neck pain with left upper limb
agonizing pain. The patient was operated 2011
elsewhere for PCD C5-6 for the same limb pain.
MRI cervical spine done 11-February-2015 showing
huge extruded disc C5-6 with left foraminal
occlusion. There is bulge of C3-4.
On examination, the patient is in agonizing pain
with weak flexion, extension right hand and the
left triceps 4/5. The patient was advised to
undergo surgery but he escaped. The patient then
came 18-March-2015 with more deterioration
urging for surgery.
Using the C-arm, and the old incision, the C6-7
level was identified. Discectomy of C6-7 was
performed and the extruded disc to the left
foramen was removed. Anatomical cage 5 was
inserted and using Atlantis one level 27 mm was
used to fuse C6-7. Through separated cleavage
the C3-4 was identified and discectomy of the
C3-4 with osteophytectomy was achieved. Using
cage 6 and one level cage same dimensions as the
previous one was used to fuse C3-4.
Smooth postoperative recovery. The power of left
upper limb became normal.
Comments
The patient patient has multiple problems
which need correction through the same incision. The upper
level was difficult to handle due to inadequate incision
projection.
