YEARS EXTRUDED DISC L5-S1 WITH LEFT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic 17-May-2015
complaining of left sciatica for 3 months with
exacerbation of the sciatica and LBP after one
week. MRI lumbar spine done 26-March-2015
showing huge extruded disc L5-S1 with left
foraminal occlusion.
On examination; the patient is limping with
exaggerated scoliotic stance. Neck and upper
limbs were neurologically free. SLRS was 45 degrees with pain in the
left. There is weak dorsiflexion
left foot -4/5 and planterflexion same foot 4/5.
The
left AJ is absent.
The patient was sent for investigations and MRI
lumbar spine performed 17-May-2015 showing
the same extrusion with bulge L4-5.
Using the C-arm, the L5-S1 level was identified.
Foraminotomy left S1 root was performed and
removal of the huge extrusion was
achieved in several pieces lateral to the axilla.
Left sided
intradiscal cleaning L5-S1 disc space. Routine
closure of the wound.
Smooth postoperative recovery. The power of left
foot became normal.
Comments
The patient still having an estimated
postoperative recurrence around 7%.
