RECURRENT DISC L4-5 WITH LEFT HUGE EXTRUSION.
Comments
There is an estimated postoperative
recurrence around 7%, because the disc space is not
completely collapsed.
Anamnesis
The patient came to the clinic
15-June-2003 complaining of LBP with left
sciatica for 2 months. he patient was operated
20 years ago and 1998 for the extruded disc with
same sciatica. MRI done recently showing huge
recurrence L4-5 with left foraminal occlusion.
On examination: the patient in agonizing pain,
limping with exaggerated scoliotic stance. SLRS
was 40 degrees left side with pain, hypalgesia
left L5 root with weak dorsi and planterflexion
left foot -4/5.
The old incision refreshed
and scarolysis with extended foraminotomy left L5
root was achieved. The extrusion was removed in one
huge piece lateral to the root of L5 and
left sided cleaning of L4-5 was performed. The
patient was put in Reverse Trendelenburg position
with Valsalva maneuver and hyperventilation. No CSF
leak. Routine closure of
the wound
Smooth postoperative recovery.
He was sent to the ward.
Follow Up
The patient came to the clinic 29-June-2003 with
clean wound and full power of the left foot.
