20-JUNE-2003 MUHAMED ALI SALEH 22 YEARS
GIANT CELL TUMOR OF THE LEFT L4-5 FACET WITH SEVERE EXTRADURAL COMPRESSION.
Anamnesis
The patient a Yemeni citizen came to the clinic
16-June-2003
complaining of LBP for 18 months with left
sciatica for 8 months. CT-scan of the lumbar
spine done 25-May-2003 showing hyperostotic
lesion left L3-4 facet joint.
On examination, the patient is limping with
scoliotic stance with SLRS 80 degrees with pain
right side and 60 degrees with more pain in the
left. There is weak dorsiflexion both feet -4/5
and planterflexion left foot 4/5.
The patient was sent for investigation and MRI
lumbar spine done 18-June-2003 showing the mass
in the left L3-4 facet severely compressing the
canal and bone scan ruled out presence of
metastasis.
Decompressive laminectomy
L3-4 with drilling of the bloody mass and
foraminotomy both L4 roots. The mass was
multiple in consistency with soft tissues and
bony tissues which were bleeding as a hill. The
histologic result was giant cell tumor.
Smooth postoperative recovery.
He was sent to the ward.
FOLLOW UP
The patient
came 28-June- 2003
with recovery of the power and sensation of both
feet. Advised to repeat MRI after 2 months, but he
disappeared.
Comments
The patient has a mass that compressing the
neural structures. Removal of the mass was to
decompress the neural structures and for
histologic verification..
