21-JULY-2003 MUHAMED FATHI MUHAMED 39 YEARS
EXTRUDED DISC L4-5 RIGHT SIDE.
Anamnesis
The patient came to the clinic 19-July-2003
complaining of LBP with right sciatica for 2
months with numbness of the III-V toes right
foot with positive cough sign. MRI lumbar
spine done 11-June-2003 showing extruded disc
L4-5 with right foraminal occlusion.
On examination, the patient is in agonizing
pain, limping with exaggerated scoliotic stance.
There is hypalgesia right S1 root with weak
dorsi and planterflexion right foot3/5. SLRS was
50 degrees with pain right side and 80 degrees
left side without pain.
The patient was operated at
Al-Hayat hospital. Right L4-5 hemiflavotomy with
right L5 foraminotomy. The extruded disc was
removed from the right side lateral to the
axilla and right sided cleaning of the disc
space of L4-5 was achieved. The patient was put
in Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak.
Routine closure of the wound.
Smooth postoperative recovery.
The power of left foot normalized. He was sent to the ward.
FOLLOW UP
The patient
was followed by the staff of Al-Hayat hospital.
Comments
The estimated recurrence rate
still ranking around 7%, because the disc space
still not shallow.
