23-MARCH-2015 MUHAMED ALI AL-HMOUD 56 YEARS
HUGE RECURRENT EXTRUSION L5-S1 WITH LEFT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 17-March-2015
complaining of LBP with agonizing left sciatica
down to the big toe left foot for one month. The
patient underwent discectomy 1994, 1997 and 1998
for the lumbar spine in Syria and fixation of
D7-8 2006 in Syria. Cath done 2008 was normal.
On examination, the patient is limping, bended
anterior with exaggerated scoliotic stance. SLRS
70 degrees with pain shooting to the left leg
and 10 degrees in the left with more pain. There
is weak dorsi and planterflexion left foot 3/5
with analgesia left L5 and hypalgesia left
S1 territories.
The patient was sent for investigations and MRI
done same day showing huge recurrence L5-S1 with
left downward migration.
Neurolysis of the left S1 root. There is massive
scar. The disc space of L5-S1 is very shallow.
The downward migrating fragment was removed in
several pieces. The root still swollen.
Subaxillary inspection ruled out the presence of
subaxillary fragments. The root was opened 2 mm
length at the posterior wall. No intradural
fragment, neither CSF flow. Routine closure of
the wound.
Smooth postoperative recovery. The power of left
foot limb became better.
Comments
The patient has
massive scars with intradural arachnoiditis with this
slipped fragment. Recovery will depend upon the degree of
the problem caused by the extrusion.
