25-JULY-2003 MUHAMED KHADER SALEM 22 YEARS
SEVERE RUPTURE OF C5-6 WITH TETRAPLEGIA.
Anamnesis
The patient came to the Emergency of Shmaisani
hospital after suffering RTA 1 day ago,
transferred from other hospital with
tetraplegia.
On examination, the patient is paraplegic with
para-aneasthesia below C3 level with dysphonia
and difficult breathing with collar in the neck
and Foley's catheter. MRI cervical spine showing
fracture far dislocation of C5-6.
The patient in supine
position with the usual skull clamps with
distraction force 10 KG was applied to reduce
fracture. Exploration of the area showing that
reduction is impossible without performing
discectomy C5-6 and removing the bony elements
obstructing the reduction. After acceptable
reduction of the bony alignments, using Cloward
instrumentation the huge sized caliber, a bone
graft was obtained from the right iliac bone and
fixed to 2 level Scientx cervical plate and
fixation of C5-6 was performed. No CSF leak,
denoting that the dura was not torn.
Routine closure of the wound.
Considering that the patient
was breathing with difficulty, he was put in
ventilator and sent to the ICU.
FOLLOW UP
The patient
was followed in the ICU and after performing check
of the cervical spine it was found that the
reduction is un-acceptable.
Comments
The condition of the patient is
grave and in case of recovery with this case he will
stay paraplegic.
