HUGE EXTRUDED DISC L4-5 WITH MIGRATION POSTERIOR TO THE DURAL SAC FROM THE
RIGHT.
Anamnesis:
The patient came to
Al-Shmaisani hospital 23-August-2009 complaining
of LBP with bilateral sciatica and weak both
feet and numbness both feet.
MRI of the lumbar spine
performed 19-August-2009 showing huge extruded
disc L4-5 with migration posterior to the dural
sac from the right.
On examination: the patient
is limping with scoliotic stance with SLRS 60
degrees both sides and weak dorsi and
planterflexion both feet 4/5.
Using image-intensifier, the
extruded disc was removed directly fro under the
removed ligamentum flavum . It was very huge and
extending to the right. Bilateral cleaning of
the disc space of L4-5 and insertion of PEEK
Satellite nucleus replacement sphere number 15
was inserted. Several image-intensifier controls
were performed.
Routine closure of the
wound and smooth
postoperative recovery
and the power of both
feet normalized.
Comments
The recurrence in this case
is ranking high, for what nucleus replacement
sphere was applied to decrease the recurrence.
It is the second case in this
month that the extruded material migrate
posteriorly behind the dural sac, which is very
rare.
