HUGE EXTRUDED DISC L4-5 WIDE BASED WITH SEVERE SEGMENTAL STENOSIS.
Anamnesis
The patient came to the clinic 27-October-2015
complaining of LBP for 3 years with intermittent
course. He got exacerbation of his LBP with
bilateral sciatica for 1 month. MRI lumbar spine
performed 01-October-2015 showing huge
wide-based extruded disc L4-5 with severe
segmental stenosis. The patient is a known
diabetic for 2 years and hypertensive for 20
years.
On examination: the patient is limping with
exaggerated scoliotic stance. SLRS was 60 degrees with pain
both
sides. There is weak dorsiflexion both feet 3/5.
The patient was sent for investigations. Dynamic studies
were unremarkable except for Bamboo spine with
calcification of the annulus fibrosis even at
the anterior border of L4-5. The patient was
sent for cardiac evaluation.
Decompressive laminectomy L4
and upper third of L5. Bilateral foraminotomy
both L5 roots with bilateral removal of the
extrusion and bilateral cleaning L4-5 disc space.
The left L5 root was severely damaged and CSF
leak was noted. Several stitches were applied
and check for CSF leak was negative. Using MultiGen, a pulsed mode
RF with 42 Celsius, 240 sec, 2 Hz and 20 mesec
duration, both L5 roots were manipulated.
Routine closure of the
wound.
Smooth postoperative recovery. The
power of both feet became normal and the patient was
sent to the ward.

Comments
The patient still has an estimated
postoperative recurrence around 7%.
This is the 19th case using the PRF mode
with MultiGen.
