27-MAY-2003 MUHAMED ISA SALMAN 48 YEARS HUGE
EXTRUDED DISC C4-5.
Anamnesis
The patient came
to the clinic 05-January-2003 complaining of
numbness both hands for 9 years for what
CT-release both hands done 1995 without improvement. Numbness of
the right foot for 18 months. The patient having
diabetes mellitus for three years. On
examination, he had severe atrophy of both
interossii with weak both upper limbs. There is
also weak dorsiflexion left foot.
MRI cervical
spine done 03-April-2003 showing showing huge
extruded disc C4-5 with malacia of the spinal
cord and elements of OPLL.
Anterior discectomy of C4-5
with removal of the extrusion was done. The dura
was seen all over the posterior aspect of the
posterior edges of the C4 and C5 bodies.
Routine closure of the wound.
Smooth postoperative recovery. The power of both
upper limbs improved dramatically.
Comments
The patient has several problems, the huge
extrusion must be removed, but the OPLL is still
in its minor character. No need to intervene
with this problem with this at the present time.
If in the future it will escalate, then it will
be dealt accordingly.
