EXTRUDED WIDE BASED DISC L4-5, MORE TO THE LEFT WITH SCIATICA MORE TO THE RIGHT.
Anamnesis
The patient came to the clinic 24-May-2015
complaining of LBP for 3 months with
exacerbation of right sciatica the last 15 days.
There is numbness IV and V toes right foot. MRI lumbar spine done 23-May-2015
showing wide based extruded disc L4-5 with
bilateral
foraminal occlusion.
On examination; the patient is limping with
exaggerated scoliotic stance. Neck and upper
limbs were neurologically free. SLRS was 30
degrees with pain right and 45 degrees with pain in the
left. There is weak dorsiflexion
both feet -4/5.
Using the C-arm, the L4-5 level was identified.
Foraminotomy L5 roots was achieved. The extruded
disc was removed from both sides and bilateral
intradiscal cleaning was performed. Routine
closure of the wound.
Smooth postoperative recovery. The power of both
feet became normal.
Comments
The patient still having an estimated
postoperative recurrence around 7%.
