RECURRENT DISC L4-5 WITH SLIPPED EPIPHYSIS.
Comments
The slipped epiphysis usually happens in
adults with age 12-18 years. Here the age of the patient is
not going with this phenomenon. But here it took place. May
be the first surgery with bilateral meticulous cleaning
triggered this process. It could be that slipping of the
epiphysis could have pace from start and it was minimal and
un-noticed.
Anamnesis
The patient was operated by me
12-April-2003 for huge central disc, for
what bilateral meticulous cleaning was done. The
patient was doing well until 1 month ago got LBP
with right sciatica. MRI requested and performed
04-June-2003 and there was only adhesions at the
operative site. The sciatica subsided but drop
left foot took place, for what he was admitted
to Shmaisani hospital and another MRI requested
and done 28-June-2003 showing a huge recurrence
of L4-5 with slipped epiphysis with downward
migration left side.
Scarolysis of the previous
surgery with maximum direction toward the left L5
root. The very huge slipped endplate was removed in
three big pieces lateral to the axilla. Exploration
of the left L5 root showed eroded lateral wall of
left L5 root dura, for what a piece of muscle was
applied to it. No CSF leak. A dual fat flaps in
pedicle were transferred from the subcutaneous layer
of the wound and pushed inside the cavity. Routine closure of
the wound
Smooth postoperative recovery.
The radicular pain decreased and the power of both
feet improved. He was sent to the ward.
Follow Up
The patient came to the clinic 26-July-2003
with clean wound with no sensory or motor
deficit. SLRS was 85 degrees both sides
Skyra MRI with all clinical applications in the run since 28-Novemeber-2013.
Inomed Riechert-Mundinger System, with three point
fixation is the most accurate system in the market. The microdrive and
its sensor gives feed back about the localization.
