SEVERE LUMBAR CANAL STENOSIS L4-5.
Anamnesis
The patient came to the clinic 24-November-2004
with LBP for 10 years with intermittent
claudication the last 7 months with progressive
course. MRI lumbar spine performed
07-November-2004 showing severe LCS L4-5. He is
using crutches for 4 years.
On examination: the patient is limping with
exaggerated scoliotic stance with right sciatica. SLRS was
90 degrees both sides. There is weak dorsi and
planterflexion both feet
3/5 with hypalgesia both feet above the ankles.
The patient was sent for cardio-consultation and
to stop anti-coagulants.
Decompressive laminectomy L4-5 with extended
foraminotomy both L5 roots was achieved with
inspection of the disc posterior border, which
was decided not to violate it. The patient was
put in Reverse Trendelenburg position with
Valsalva maneuver and hyperventilation. No CSF
leak. Routine closure of the wound.
Smooth postoperative recovery and the power of
both feet normalized and the patient
was sent to the ward.
Follow Up
The patient came to the clinic 15-December-2004
with clean wound and normal power of both feet
with hypalgesia descended to the big toes both
feet.
Comments
The sooner the surgical intervention, the better
the outcome, because stenosis is a progressive
one.
