HUGE EXTRUDED DISC L3-4 MORE TO THE RIGHT WITH COMPLETE OCCLUSION OF THE
CANAL.
Anamnesis
The patient came to the clinic 25-January-2015
complaining of LBP for 5 months with bilateral sciatica
more the left down to the gastrocnemius muscles
both sides. MRI
lumbar spine done 27-August-2014 showing
broad based extruded disc L3-4 more to the left.
On examination, the patient is in agonizing pain, limping with exaggerated scoliotic
stance. SLRS was 10 degrees with pain in the
right and 5 degrees with pain in the left. There is weak dorsiflexion right foot
-3/5, planterflexion same foot -4/5.
Dorsiflexion left foot -4/5. the right
quadriceps muscle power 4/5.
The patient was sent for new investigations. MRI
dorsal showed small PDD D7-8 and D11-12 of no
clinical significance. MRI lumbar spine showed
very huge extruded disc L3-4 with complete
occlusion of the canal with downward migration
more to the right.
Using C-arm the level of L3-4 was identified and
laminectomy of 3/4 of L3 and upper third of L4
was done. Foraminotomy both L4 roots was performed. The
extruded disc was removed lateral to the axilla
and the downward migrating piece was removed
from both sides and
bilateral intradiscal cleaning was done.
Routine closure of the wound.
Smooth postoperative recovery. The power of both
legs became normal.
Comments
There is still an estimated postoperative
recurrence around 7%.
