L5-S1 LEFT SIDE.
Anamnesis
The patient came to the clinic 29-September-2007
complaining of LBP and left sciatica for 45
days. MRI lumbar spine done 02-September-2007
showing extruded disc L5-S1 left side. All
conservative treatment measures were performed,
without improvement.
SLRS was 30 degrees in the left with agonizing
pain, with hypalgesia left L5 root territory and
weak dorsi and planterflexion left foot.
Using Inomed Highline IOM, the operation was
performed. Foraminotomy of the left S1 root was
performed and left 1/3 flavotomy L5-S1 achieved.
The extruded disc was removed lateral to the
axilla with left-sided cleaning of the disc
space.
Smooth postoperative recovery and improvement of
the power of left foot.

Comments
The patient is 120 Kg weight and the disc
space still high. These parameters increase the expected
recurrence rate.
The disc was removed from a small hole in the
annulus fibrosis to decrease the possible recurrence.
The epidural fat was preserved and reflected
back around the root to prevent future fibrosis around the
involved root.
Inomed Highline IOM is applied to catch all
the possible complications and it will be introduced in practice
as the ECG monitor in anesthesia or ICU patient.
