30-OCTOBER-2013 MUNA MUSA ABU-RSHEYD 49 YEARS
EXTRUDED DISC L4-5 WITH LEFT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic 19-September-2013
complaining of LBP for 6 years with exacerbation
of LBP and left sciatica last year down to the
II and III toes left foot.
MRI lumbar spine done 19-January-2013 showing
extruded disc L4-5 with left downward migration.
On examination: the patient is limping with exaggerated scoliotic
stance. SLRS was 65 degrees with shooting pain
in the left. Weak dorsiflexion left foot
4/5
and planterflexion 4/5.
The patient was sent for new MRI and done
22-September-2013 showing extruded disc L4-5
with left foraminal occlusion.
Foraminotomy left L4-5 after using C-arm. The
root was severely compressed by the extrusion with atrophic
changes of the root. The extruded disc was
removed lateral to the axilla. Left sided
cleaning L4-5 disc space.
Smooth postoperative
recovery. The power of the left foot became
normal.
Comments
The patient still have an estimated
postoperative recurrence rate around 7%.
