HUGE PLD L4-5 WITH RIGHT DOWNWARD MIGRATION.
Anamnesis
The patient
came to the clinic 21-March-2011 complaining of
right sciatica for one week and agonizing LBP
for several hours.
MRI
lumbar
spine performed 21-March-2011 showing extruded
disc L4-5 with right downward migration.
On
examination: the patient is limping with
exaggerated scoliotic stance. SLRS was 20
degrees in the right with pain. There is weak
dorsi and planterflexion right foot 3/5.
Right L5 foraminotomy with removal of the
extrusion lateral to the axilla in several
fragments. Right sided cleaning of L4-5 disc
space
Routine
closure of the wound. Smooth postoperative
recovery with improvement of the power of right
foot.

Comments
The expected recurrence rate still around 7%
because, the disc space height still not
shallow.
It was necessary to remove the intradiscal
material, because the extruded disc fragments
were severely involved with inflammatory
process, which means, that the intradiscal
material is the same.
