22-JANUARY-2014 MURAD AQEL WAHDAN 26 YEARS EXTRUDED
DISC L5-S1 WITH RIGHT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 19-January-2014
complaining of LBP and right sciatica for 18
months. Exacerbation last 3 months. MRI lumbar
done 18-October-2012 showing small central
extruded disc L5-S1.
On examination; the patient is not limping with
exaggerated scoliotic stance. SLRS was 30
degrees with pain in the right. The right AJ is
absent in comparison to the left. There is gross
weak dorsi and planterflexion right foot 3/5
with hypalgesia right L5 territory.
The patient sent for new
investigations and MRI lumbar spine done
19-January-2014 showing extruded disc L5-S1 with
right downward migration.
Using C-arm, the L5-S1 level
identified. Right S1 root foraminotomy. Removal
of the extrusion lateral to the axilla. Right
sided intradiscal cleaning L5-S1 disc space.
Smooth postoperative
recovery. The power of the right foot became
normal and the right sciatica disappeared.
Comments
The patient still has an estimated postoperative
recurrence around 7%, because the disc space is
still not shallow.
