The patient was operated several years ago for PLD L4-5 left side. The patient improved slightly but continued to complain and the patient  progressed drop right foot for three months and MRI showed huge recurrence at the left side without causing any manifestation in the left, but with relatively small fragment migrating down to the right L5 root , causing the drop right foot . Bilateral cleaning of the disc space was performed  with bilateral foraminotomy of both L5 roots with removal of all the extrusions. During the skeletonisation a pinpoint tear of the dura  in the left side happened at the site where it is not expected to have dural elements. The tear was closed by nyelon 5 Zero. Valsalva maneuover was negative. The patient was diabetic and was discharged 06-June-2005 with normalisation of his neurologic condition.
