DISC L2-3.
Anamnesis
The patient came to the
clinic 05-August-2004 complaining of LBP with
left sciatica for more than 5 years with
intermittent course with agonizing LBP with
left sciatica
for 3 weeks, down to L4 root
territory. MRI lumbar spine
done 04-August-2004 showing huge extruded disc
L2-3 with left foraminal occlusion. He has
arterial hypertension for 5 years under
treatment.
On examination, the patient in agonizing pain,
limping with exaggerated scoliotic stance with
weak dorsiflexion left foot 3/5
and planterflexion left foot. SLRS was 70
degrees with pain left side and 85 degrees
right side with
pain.
Foraminotomy left L2 and L3
roots was achieved.
Left sided intradiscal cleaning L2-3 was
performed. The left L2 root was compressed and
released and it was possible to see it in the
far up left corner of the field. The patient was put in Reverse
Trendelenburg position with Valsalva maneuver
and hyperventilation. No CSF leak. The epidural
fat was transferred to the left axilla area. A fat was
harvested from the
subcutaneous layer with pedicle and transferred and covered the exposed parts
of the root to minimize postoperative
scarring. Routine closure
of the wound.
Smooth postoperative recovery. The power of
the left foot normalized.
FOLLOW UP
The patient
came to the clinic 19-August-2004 with clean wound and no motor,
nor sensory deficit.
Comments
There is still an
estimated postoperative recurrence around 7%,
because the disc space is still not completely
collapsed.
