17-JANUARY-2005 MUSA MUSTAFA MUSA 34 YEARS EXTRUDED DISC L4-5 WITH LEFT DOWNWARD
MIGRATION.
Anamnesis
The patient came to the clinic 31-October-2004
complaining of LBP for 10 years with left
sciatica the last 2 months . The patient was not
limping with SLRS 80 degrees with pain in the
left.
On examination: The patient is in pain with
exaggerated scoliotic stance. There is weak
dorsiflexion left foot -4/5. There is hypalgesia left L5, S1
territories.
The patient was sent for investigations and MRI
lumbar spine performed 05-November-2004 showing
huge extrusion L4-5.
Left extended L5 foraminotomy
with removal of the extrusion from under the axilla
and lateral to it. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak. Routine
closure of the wound.
Smooth postoperative recovery.
The power of the left foot improved.
He was sent to the ward.
Follow Up
The patient came to the clinic 30-January-2005
with SLRS 90 degrees in the left with
disappearance of hypalgesia and little
improvement of the power of the left foot.
Comments
There is still an estimated postoperative
disc recurrence around 7%, since the disc space is not
completely shallow.
