The patient came to the clinic 03-August-2014 with history of neck and both shoulders pain 1 year ago. 4 months ago during entrance, the forehead was stuck to the transparent door from glass and he got cut wound to the left eyebrow. After that he started to suffer from severe neck pain with numbness of the I and II fingers both hands with cephalic pain and awakening him during night. He noticed weak both upper limbs with fainting attacks.

On examination; the patient has neck pain when looking to all directions, more to the left. Romberg was stable with no cerebellar signs. There is weak grip and extension both hands -4/5 with weak both triceps muscles -3/5. There is hypalgesia starting from the lower edge of the left jaw down to the left D2. There is dyseasthesia with hypalgesia below the left knee.

The patient was sent for investigations: MRI brain done 05-August-2014 was normal. MRI cervical spine showed a huge extruded disc C4-5 with malacia of the spinal cord at this level.

Using C-arm, the C4-5 level was identified. Discectomy C4-5 was done until the dura was seen all over. Solis cage 14x5x12 mm was inserted with Vitos bone graft. Cervical plate one level 16 mm length was used with 4 variable screws to fuse C4-5. Routine closure of the wound.

Smooth postoperative recovery with dramatic improvement of the power of both upper limbs.

The patient suffered whiplash injury to the neck, which triggered the already involved segment.
