HUGE EXTRUDED DISC L4-5 LEFT SIDE.
Anamnesis
The patient a Saudi citizen came to the clinic

complaining of LBP with bilateral sciatica more
to the left for 4 months. Positive cough sign.
MRI lumbar spine done 26-August-2002 showing
huge disc L4-5 with left downward migration.
On examination: The patient in agonizing pain,
limping with exaggerated scoliotic stance.
SLRS
30 degrees
with pain in the left and 90 degrees in the
right. There is severe weak dorsi planterflexion
left foot 3/5, with hypalgesia left
L5 territory.
Left sided L5 foraminotomy with removal of the
extrusion lateral to the axilla from the
right. Left sided intradiscal cleaning L4-5. The
left downward migrating piece was removed. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak.
Routine closure of the wound.
Smooth postoperative recovery.
The power of the left foot improved.
He was sent to the ward.
Follow Up
The patient came to the clinic 24-November-2002
with clean wound and still having weak
dorsiflexion left foot 4/5 with
SLRS 70 degrees without pain.
The patient then came 30-August-2005 with left
sciatica for 6 months and signs Parkinson's
disease and given Senimet and sent for
investigations, but he disappeared.
Comments
There is still an estimated postoperative
recurrence around 7%, because the disc space is still not
shallow.

Inomed Riechert-Mundinger System, with three point
fixation is the most accurate system in the market. The microdrive and
its sensor gives feed back about the localization.
