11-DECEMBER-2012 MUTAZ MUHAMED SAAD 42 YEARS
EXTRUDED DISC L4-5 WITH RIGHT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 10-December-2012
complaining of LBP for 2 years with right sciatica for 4
months with pain and numbness all toes right
foot. Using crutches for 5 months. The patient
has squint left eye since childhood. The patient
performed MRI lumbar spine 22-October-2011
showing bulge L4-5 right side.
On examination: the patient is limping
in agonizing pain, dragging the right leg with
exaggerated scoliotic stance. SLRS 5 degrees
right side with pain. There is
weak planter and dorsiflexion right foot -4/5.
MRI of the lumbar spine done
10-December-2012
showing extruded disc L4-5 with right downward
migration.
Right L4-5 hemiflavotomy with
foraminotomy right L5 root. The extruded disc
was attacked lateral to the axilla and removed
in one big piece. Right sided cleaning L4-5 disc
space.
Routine closure of the wounds. Smooth
postoperative recovery. The power of the right
foot
became normal.

Comments
The patient still has
an estimated postoperative recurrence rate
around 7%, because the disc space is still not
shallow.
