YEARS EXTRUDED DISC L4-5 WITH RIGHT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic
29-December-2013 complaining of LBP with right sciatica
for 2 years with intermittent course with
exacerbation last 15 days. MRI lumbar spine done
28-December-2013 showing extruded disc L4-5 with
right downward migration. Dynamic studies were
normal, but reported as having retrolisthesis
same level.
On examination: the patient is limping with
scoliotic stance. SLRS was 40
degrees right side with pain. There is almost
drop right foot with weak dorsiflexion -3/5.
Using C-arm, the L4-5 level
was identified. Right L4-5 hemiflavotomy done
and foraminotomy of the right L5 root achieved.
The extruded downward migrating extrusion was
removed lateral to the axilla in one piece.
Right sided intradiscal cleaning L4-5 disc
space.
Smooth postoperative
recovery. The power of the right foot became
normal.
Comments
The estimated postoperative third recurrence of L3-4
is still around 7%, because the disc space is
still not completely shallow.
