The
patient came to the clinic 16-April-2011
complaining of exacerbation of left sciatica for
4 days . |

The
patient performed MRI lumbar spine 16-April-2011
showing recurrent PLD L5-S1 with left downward
migration and Modic type II changes. |

On
examination: the patient is in agonizing pain
and unable to be examined. He had weak dorsi and
planterflexion left foot 4/5.

The
patient was operated by me 29-December-2003 for
PLD L5-S1 left side. |
|
The old wound refreshed and foraminotomy of the
left S1 root was performed and scarolysis was
done to achieve approach to the extrusion
lateral to the axilla. The extrusion was removed
and inspection for migrating remnants was
performed subaxillary and the foraminotomy was
extended 10 mm below the axilla. The root was
swollen and thick in consistency.

closure of the wound. Smooth postoperative
recovery with improvement of the power of left
foot. |
