YEARS HUGE EXTRUDED DISC L5-S1 MORE TO THE LEFT.
Anamnesis
The patient's family came to the clinic 27-November-2004
with telling that the patient is in agonizing LBP and left sciatica for 20 days. MRI lumbar spine performed
18-November-2004 showing huge extruded disc
L5-S1 more to the left with downward migration. The patient did not come
to the clinic due to agonizing pain.
On examination before surgery: the patient is in
bed with agonizing
pain. There is weak dorsi and planterflexion left foot
2/5.
Left L5-S1 hemiflavotomy with foraminotomy left
S1 root was achieved. Most of the the extruded disc was
removed lateral to the axilla. Another part was
removed subaxillary. Bilateral cleaning L5-S1
was achieved. The patient was
put in Reverse Trendelenburg position with
Valsalva maneuver and hyperventilation. No CSF
leak. Routine closure of the wound.
Smooth postoperative recovery and the power of
the left foot normalized and the patient
was sent to the ward.
Follow Up
The patient came to the clinic 13-December-2004
with clean wound and normal power of the left
foot.
Comments
There is still an estimated postoperative
disc recurrence around 7%, since the disc space is not
completely shallow.
