EXTRUDED DISC C6-7 LEFT SIDE.
Anamnesis
The patient came to the
clinic 24-April-2004 complaining of neck pain
for 15 years. MRI cervical spine performed
22-November-1998 showing extruded disc C6-7,
more to the left. The last month got left upper
limb pain. Seen by cardiologist. Not diabetic,
but using L-thyroxin 75 microgram daily with
arterial hypertension for 5 months in treatment.
On examination, the patient
in agonizing pain, having neck pain when turning
the head to all directions. There is severe weak
grip and extension left hand 3/5 and left
triceps muscle 3/5.
The patient sent for
investigations and MRI cervical spine done
26-April-2004 showing extruded disc C6-7 with
occlusion left side.
Using the C-arm, the level of
C6-7 was identified and discectomy of C6-7 was
performed and the extruded pieces in the left
side were removed until the dura was seen at the
bottom of the removed disc cavity. Routine closure
of the wound.
Smooth postoperative recovery. The power of the
left upper limb dramatically improved.
FOLLOW UP
The patient
came to the clinic 17-May-2004 with clean wound
and neurologically free.
The patient
then came several times, the last one 08-November-2004
during Ramadan with right upper limb pain and she
was neurologically free and treated conservatively.
Comments
The recurrence rate
in cervical disc surgery is zero. Another disc
can cause de novo extrusion.
Skyra MRI with all clinical applications in the run since 28-Novemeber-2013.
Inomed Riechert-Mundinger System, with three point
fixation is the most accurate system in the market. The microdrive and
its sensor gives feed back about the localization.
