LCS L2-3, 3-4, L4-5 WITH SPONDYLOLISTHESIS L2-3, L4-5 WITH WITH EXTRUDED DISC
L3-4 LEFT SIDE.
Anamnesis
The patient came to the clinic 23-June-2014
complaining of LBP for
4 years with left sciatica for 3 months with
exacerbation last 15 days with numbness both
feet more the left. MRI lumbar spine done
23-June-2014 showing spondylolisthesis L2-3,
L4-5 with segmental stenosis at L2-3,
L3-4, L4-5 with extruded disc L3-4 with left
foraminal occlusion.
On examination; the patient is limping with exaggerated scoliotic stance.
SLRS was 50 degrees with pain in the left and 60
degrees with pain in the right. There is weak
dorsiflexion both feet -4/5 and planterflexion
both feet 4/5. There is hypalgesia left L4,5 and
S1 roots territories.
Decompressive laminectomy L3, lower half of L2
and upper third of L4. Foraminotomy of left L4
root. Using XIA 3 Stryker transpedicular screws
monoaxial screws were inserted to the L2, L3 and
L5 levels. 2 polyaxial screws were inserted to
L4 level. Using bended rods fusion of L2.3.4 and
L5 levels was achieved. Cross connector
MAC 42 mm was used at the level of L3-4. The
harvested bone was melt by Midas Rex milling
machine and put lateral to the rods both sides.
Routine closure of the wound.
Smooth postoperative recovery.
Comments
The patient has several problems in the
lumbar spine and all of them are blamed with here complains.
They were all corrected.
