27-JUNE-2014 NAIM ABDEL-KAREEM DAAS 52 YEARS
HUGE EXTRUDED DISC L3-4 WITH SEVERE SEGMENTAL STENOSIS.
Anamnesis
The patient came to the clinic 19-June-2014
complaining of LBP for 15 years. Numbness left
leg for one year with bilateral numbness and
inability to walk more than 150 meters with
progressive deterioration.
On examination: the patient is not limping with
no scoliotic stance. SLRS was 90 degrees without
pain both sides. Weak dorsiflexion right foot
4/5 and left foot 3/5.
The patient sent for investigations and done
23-June-2014. MRI lumbar spine showing huge
extruded disc L3-4 with severe segmental
stenosis.
Using C-arm, the L3-4 level was identified and
bilateral foraminotomy of L4 roots was achieved.
Bilateral removal of the huge extrusion was done
with bilateral intradiscal cleaning of L3-4 disc
space.
Smooth postoperative recovery. The
power of the right foot became normal and the
power of the left foot became 4/5.
Comments
There is still an estimated postoperative
recurrence around 7%, because the disc space height still
not completely shallow.
